The Cleveland Clinic has agreed to stop providing transgender medical treatments to minors for 20 years as part of a settlement with the Justice Department and the state of Ohio, according to a Justice Department announcement Monday.

Associate Attorney General Stanley Woodward said in the department's statement that the agreement advances the Trump administration's efforts to protect children from what it calls irreversible medical interventions and ensures compliance with federal law.

Under the settlement, the Cleveland Clinic agreed not to provide puberty blockers, cross-sex hormones, or other medical gender-transition interventions to patients under age 18 for the next two decades, the Justice Department said.

The department also said the health system will contribute $2 million to support medical care for detransitioners, or individuals seeking treatment after previously undergoing gender-transition interventions.

In addition, the Cleveland Clinic agreed to pay $308,000 to resolve allegations involving Medicaid billing practices tied to treatments provided to minors, according to the Justice Department.

The Cleveland Clinic said the billing issue stemmed from what it described as an unintentional coding error affecting a limited number of patients and said it cooperated with investigators to resolve the matter.

The settlement comes despite Ohio already restricting gender-transition treatments for minors under state law.

According to the Justice Department, the agreement will remain in effect for 20 years regardless of whether Ohio's restrictions are later modified or overturned.

Woodward said in the department's announcement that the settlement reflects the administration's commitment to ensuring that children are not subjected to life-altering medical procedures before reaching adulthood.

The agreement follows a similar settlement announced by the Justice Department last month involving Texas Children's Hospital, which federal officials said included financial penalties and the establishment of services for detransitioning patients.

The Trump administration has made investigations of pediatric gender-transition treatments a priority, arguing that puberty blockers, cross-sex hormones and related procedures can carry significant long-term consequences for minors.

Supporters of such treatments contend they can benefit some patients experiencing gender dysphoria, while administration officials have argued that children are often too young to provide meaningful informed consent for interventions with potentially permanent effects.

The Cleveland Clinic settlement marks one of the most significant agreements reached so far as the administration expands scrutiny of hospitals and medical providers that offer gender-transition treatments to minors.
